Ethnic minority populations with darker skin are considered as a high-risk group for vitamin D deficiency, owing mainly to having less ability to synthesise vitamin D from sunlight due to the skin pigment melanin and/or reduced skin exposure due to clothing required by religious or cultural traditions. The aim of this literature review was to evaluate vitamin D intake and status of ethnic minority populations with darker skin. The authors examined previous observational studies and randomised controlled trials. Ethnic minority populations generally have a lower vitamin D status than white populations. Compared with Caucasians, there is evidence that Asians require approximately three times longer periods of sunlight exposure, and Africans six times the same exposure, to achieve the same blood levels of vitamin D. The main food sources for dietary vitamin D intake were different for ethnic minority populations and white populations. There is limited evidence on the impact of vitamin D supplementation on different ethnic groups. The authors concluded that food fortification could be explored to increase dietary vitamin D intake in ethnic minority populations who generally have lower levels of vitamin D.
Abstract
In recent years, vitamin D deficiency has attracted attention worldwide. Especially many ethnic minority populations are considered at high-risk of vitamin D deficiency, owing to a lesser ability to synthesis vitamin D from sunlight (ultraviolet B), due to the skin pigment melanin and/or reduced skin exposure due to coverage required by religious and cultural restrictions. Therefore, vitamin D intake from dietary sources has become increasingly important for many ethnic minority populations to achieve adequate vitamin D status compared with the majority of the population. The aim of the study was critically evaluate the vitamin D intake and vitamin D status of the ethnic minority populations with darker skin, and also vitamin D absorption from supplements and ultraviolet B. Pubmed, Embaase and Scopus were searched for articles published up to October 2018. The available evidence showed ethnic minority populations generally have a lower vitamin D status than the majority populations. The main contributory food sources for dietary vitamin D intake were different for ethnic minority populations and majority populations, due to vary dietary patterns. Future strategies to increase dietary vitamin D intake by food fortification or biofortification needs to be explored, not only for the majority population but more specifically for ethnic minority populations who are generally of lower vitamin D status.

Reactive hypoglycaemia (RH) is a condition where blood sugar levels drop too low 2-5 hours after eating. There are 3 types of RH, depending on the time course: 1. Alimentary: within the first 2 hours, 2. Idiopathic: at around 3 hours and 3. Late: around 4-5 hours, after eating. RH is tightly connected with insulin secretions. Insulin is released in 2 phases, a rapid initial release within 10 minutes of eating, and a slow release over 24 hours. Alimentary RH is usually due to rapid gastric emptying, for example after bariatric surgery (surgery for weight loss resulting in a smaller stomach volume). This leads to rapid increase in blood glucose triggering an increased insulin secretion which in turn leads to hypoglycaemia. Idiopathic RH is most commonly seen in teenagers and lean people and is not thought to be a risk factor for developing type 2 diabetes. The causes are not clear. Late RH is due to a decreased first phase insulin release which leads to increased blood glucose levels which in turn trigger an excessive late phase insulin secretion. Over time this can lead to reduced insulin sensitivity and is a risk factor for developing type 2 diabetes. This review discusses the underlying physiological imbalances and reactive hypoglycaemia as a risk factor for developing type 2 diabetes in detail.
Abstract
Reactive hypoglycemia (RH) is the condition of postprandially hypoglycemia occurring 2-5 hours after food intake. RH is clinically seen in three different forms as follows: idiopathic RH (at 180 min), alimentary (within 120 min), and late RH (at 240-300 min). When the first-phase insulin response decreases, firstly, blood glucose starts to rise after the meal. This leads to late but excessive secretion of the second-phase insulin secretion. Thus, late reactive hypoglycemia occurs. Elevated insulin levels also cause down-regulation of the insulin post-receptor on the muscle and fat cells, thus decreasing insulin sensitivity. The cause of the increase in insulin sensitivity in IRH at 3 h is not completely clear. However, there is a decrease in insulin sensitivity in late reactive hypoglycaemia at 4 or 5 hours. Thus, patients with hypoglycemia at 4 or 5 h who have a family history of diabetes and obesity may be more susceptible to diabetes than patients with hypoglycemia at 3 h. We believe that some cases with normal glucose tolerance in OGTT should be considered as prediabetes at <55 or 60 mg/dl after 4-5 hours after OGTT. Metformin and AGI therapy may be recommended if there is late RH with IFG. Also Metformin, AGİ, TZD, DPP-IVInhibitors, GLP1RA therapy may be recommended if there is late RH with IGT. As a result, postprandial RH (<55 or 60 mg/dl), especially after 4 hours may predict diabetes. Therefore, people with RH along with weight gain and with diabetes history in the family will benefit from a lifestyle modification as well as the appropriate antidiabetic approach in the prevention of diabetes.

The Mediterranean diet is considered one of the most studied diets in scientific literature and this review specifically looks at two fluid aspects of the MedDiet; olive oil and red wine. Olive oil is rich in phenolic compounds and red wine in polyphenols and the study looks at their therapeutic effect on cardiovascular disease prevention, particularly on lipids, blood pressure, plaque and glucose metabolism. Known mechanisms of the MedDiet include reduction of inflammatory and oxidative stress markers, and an improvement in lipid profile and insulin sensitivity. Polyphenols are important antioxidants abundant in plant foods including olives and red grapes used in wine (known to be x10 richer in polyphenols than white wine). The review reports that low to moderate consumption of red wine 30-50g daily lowers risk factors for CVD, improve HDL lipid profile, exerts a beneficial effect on blood pressure (BP), promotes vasodilation thus helping to reduce plaques and finally limited data shows it may beneficially affect insulin resistance. Polyphenols in olives were reported to reduce blood pressure, reduce LDL lipids and increase HDL lipids, support weight loss and help prevent obesity, metabolic syndrome and type II diabetes, reduce inflammation and oxidative stress, and possibility benefit gut microbiota. The review concludes that both fluids exert cardio-protection when consumed in moderation as part of a MedDiet.
Abstract
A growing interest has emerged in the beneficial effects of plant-based diets for the prevention of cardiovascular disease, diabetes and obesity. The Mediterranean diet, one of the most widely evaluated dietary patterns in scientific literature, includes in its nutrients two fluid foods: olive oil, as the main source of fats, and a low-to-moderate consumption of wine, mainly red, particularly during meals. Current mechanisms underlying the beneficial effects of the Mediterranean diet include a reduction in inflammatory and oxidative stress markers, improvement in lipid profile, insulin sensitivity and endothelial function, as well as antithrombotic properties. Most of these effects are attributable to bioactive ingredients including polyphenols, mono- and poly-unsaturated fatty acids. Polyphenols are a heterogeneous group of phytochemicals containing phenol rings. The principal classes of red wine polyphenols include flavonols (quercetin and myricetin), flavanols (catechin and epicatechin), anthocyanin and stilbenes (resveratrol). Olive oil has at least 30 phenolic compounds. Among them, the main are simple phenols (tyrosol and hydroxytyrosol), secoroids and lignans. The present narrative review focuses on phenols, part of red wine and virgin olive oil, discussing the evidence of their effects on lipids, blood pressure, atheromatous plaque and glucose metabolism.

Sarcopenia is a multifactorial process in which losses occur in both muscle mass and function. Although several studies indicate an association between sarcopenia and metabolic syndrome (MetS), the connection has not been extensively reviewed. The aim of this study is to examine the relationship between sarcopenia and MetS to better understand the mechanisms underlying disease and assess current therapeutic options. According to the existing literature, this study found insulin resistance, inflammation and obesity to be major underlying factors of MetS-related sarcopenia. Based on this information, the authors suggest the best option for managing MetS-related sarcopenia is preventative lifestyle change around diet and exercise until a consensus on a therapeutic treatment can be established.
Abstract
Although there are several reviews that report the interrelationship between sarcopenia and obesity and insulin resistance, the relation between sarcopenia and the other signs that compose the metabolic syndrome (MetS) has not been extensively revised. Here, we review the mechanisms underlying MetS-related sarcopenia and discuss the possible therapeutic measures proposed. A vicious cycle between the loss of muscle and the accumulation of intramuscular fat might be associated with MetS via a complex interplay of factors including nutritional intake, physical activity, body fat, oxidative stress, proinflammatory cytokines, insulin resistance, hormonal changes, and mitochondrial dysfunction. The enormous differences in lipid storage capacities between the two genders and elevated amounts of endogenous fat having lipotoxic effects that lead to the loss of muscle mass are discussed. The important repercussions of MetS-related sarcopenia on other illnesses that lead to increased disability, morbidity, and mortality are also addressed. Additional research is needed to better understand the pathophysiology of MetS-related sarcopenia and its consequences. Although there is currently no consensus on the treatment, lifestyle changes including diet and power exercise seem to be the best options.

Artificial sweeteners, such as aspartame, neotame, saccharin, sucralose, and stevia are widely promoted as low-calorie alternatives to sugar and are known as non-nutritive sweeteners (NNS). Generally, they have been considered as a healthy option to replace sugars, but data is emerging that they may influence obesity and metabolic syndrome (METs) and contribute to the development of type II diabetes. These non-nutritive sweeteners can be thousands of times sweeter than sugar and have been widely adopted by the food industry to help reduce calories, and promote weight loss and diabetic products. It is believed that 25% of children and 41% of adults consume low-calorie sweeteners regularly, with the beverage industry relying heavily on them. However, it is now been shown that these sweeteners can cause imbalances to gut bacteria and interact with taste receptors and insulin signalling. These findings mean that artificial sweeteners may trigger the same hormonal response as sugar by releasing insulin and overtime lead to insulin resistance, obesity, and overall metabolic syndrome. Finally, there is evidence that our body develops a learned response to sweeteners which paradoxically leads to weight gain.
Abstract
Individuals widely use non-nutritive sweeteners (NNS) in attempts to lower their overall daily caloric intake, lose weight, and sustain a healthy diet. There are insufficient scientific data that support the safety of consuming NNS. However, recent studies have suggested that NNS consumption can induce gut microbiota dysbiosis and promote glucose intolerance in healthy individuals that may result in the development of type 2 diabetes mellitus (T2DM). This sequence of events may result in changes in the gut microbiota composition through microRNA (miRNA)-mediated changes. The mechanism(s) by which miRNAs alter gene expression of different bacterial species provides a link between the consumption of NNS and the development of metabolic changes. Another potential mechanism that connects NNS to metabolic changes is the molecular crosstalk between the insulin receptor (IR) and G protein-coupled receptors (GPCRs). Here, we aim to highlight the role of NNS in obesity and discuss IR-GPCR crosstalk and miRNA-mediated changes, in the manipulation of the gut microbiota composition and T2DM pathogenesis.

Gestational diabetes mellitus (GDM) is an increasing public health concern that affects approximately 5-20% of pregnancies with rising prevalence. The potential impact of specific dietary interventions on the gut bacteria composition and function are of considerable interest to prevent and treat GDM. The aim of the study was to analyse the changes in the gut microbiota and the diet-microbiota interactions occurring during healthy pregnancies and pregnancies complicated by GDM. This study is a systemic review. Literature shows that pregnancies complicated with GDM may have an impaired gut microbiota, and this microbiota can be transmitted to the offspring. Diets can shape the gut microbiota, in fact dietary changes can rapidly change the gut microbiota. However, it generally reverts to the original status with short-term dietary modifications. Authors conclude that the optimal nutritional strategy in GDM patients remains unresolved. It is important that the potential benefits of diet are taken into consideration.
Abstract
Medical nutritional therapy is the first-line approach in managing gestational diabetes mellitus (GDM). Diet is also a powerful modulator of the gut microbiota, whose impact on insulin resistance and the inflammatory response in the host are well known. Changes in the gut microbiota composition have been described in pregnancies either before the onset of GDM or after its diagnosis. The possible modulation of the gut microbiota by dietary interventions in pregnancy is a topic of emerging interest, in consideration of the potential effects on maternal and consequently neonatal health. To date, very few data from observational studies are available about the associations between diet and the gut microbiota in pregnancy complicated by GDM. In this review, we analyzed the available data and discussed the current knowledge about diet manipulation in order to shape the gut microbiota in pregnancy.

The aim of the study was to review the effects of probiotics on glycaemic outcomes in type 2 diabetes or obesity. The study is a meta-analysis of RCTs that utilized probiotic yoghurt as a main intervention in participants with type 2 diabetes or obesity. Nine studies were included in the meta-analysis, seven of which were conducted in subjects with type 2 diabetes and the rest were conducted in subjects with obesity. Results indicate that probiotic yoghurt provides no significant improvement compared with the control in glycaemic markers. Authors conclude that their study did not demonstrate the benefits of consuming probiotic yoghurt compared with conventional yoghurt for improving glucose control in patients with type 2 diabetes or obesity.
Abstract
Probiotic yogurt is suggested as a nutritional approach in type 2 diabetes (T2D) and obesity. We performed a systematic review and meta-analysis of randomized controlled trials (RCTs) evaluating the effects of probiotic yogurt on glycemic outcomes in T2D or obesity. The databases used to search for RCTs included Medline and Scopus. The RCTs were eligible if outcomes included selected glycemic markers. In nine eligible trials, 237 and 235 subjects were in treatment (probiotic yogurt) and control (mostly conventional yogurt) groups, respectively. There was no significant difference for pooled unstandardized mean difference (USMD) hemoglobin A1c (HbA1c) by probiotic yogurt compared with the control in T2D (USMD: -0.366; 95% CI: -0.755, 0.024, p = 0.066) and obesity (USMD: 0.116, 95% CI: -0.007, 0.238, p = 0.065). Similarly, there were no effects of probiotic yogurt on fasting blood glucose, fasting insulin, or insulin resistance (estimated by homeostatic model assessment of insulin resistance (HOMA-IR)) in either T2D or obesity. In conclusion, the present meta-analysis has not demonstrated the benefits of consuming probiotic compared with conventional yogurt for improving glucose control in patients with diabetes or obesity. Larger trials are needed to verify the benefits of probiotic and/or conventional yogurt or other probiotic fermented milk (e.g., kefir) on glycemic markers in patients with diabetes and obesity.

Intermittent fasting (IF) is an eating pattern that cycles between periods of fasting and eating. IF has gained popularity in recent years with people wanting to lose weight, and it may have many long-term health benefits. In this review, the authors looked at human and animal studies using variations of IF including time restricted eating (TRE), where eating is confined within a specific window during the day (for example 8 hours eating and 16 hours fasting), and alternate day fasting (ADF), where a day of eating normally is alternated with a day of either fasting entirely, or significant calorie restriction. The authors found that IF is related to many beneficial effects on the cardiovascular system, involving atherosclerosis progression, benefits for diabetes mellitus type 2 such as improved glucose metabolism and insulin sensitivity, lowering of blood pressure, and other cardiovascular risk factors (such as lipid profile and inflammation). It is currently unclear whether the benefits of IF are solely due to weight loss or other mechanisms. The success of every type of diet depends on compliance, and IF seems to be as easy or easier to follow than more traditional diets for many people. Fasting is not recommended for people with hormonal imbalances, pregnant and breastfeeding women, and diabetics. People with eating disorders and underweight people are also not recommended to use the intermittent fasting diet. In recent years, the IF diet and its varieties have become increasingly popular. This diet not only serves to reduce body weight but seems to have other long-term health benefits. However, individuals’ current health and situation should be considered before commencing the IF diet.

Intermittent fasting is a form of time restricted eating (typically 16 h fasting and 8 h eating), which has gained popularity in recent years and shows promise as a possible new paradigm in the approach to weight loss and the reduction of inflammation, and has many potential long term health benefits. In this review, the authors will incorporate many aspects of fasting, mainly focusing on its effects on the cardiovascular system, involving atherosclerosis progression, benefits for diabetes mellitus type 2, lowering of blood pressure, and exploring other cardiovascular risk factors (such as lipid profile and inflammation).

The researchers reviewed specific traits including food acceptance, food intake, picky eating, appetitive traits, eating compensation, eating in the absence of hunger, and meal-specific microstructural patterns (e.g., bite rate and eating speed). They found that girls typically like a wider variety of foods than boys and had a greater preference for plant foods whilst boys typically preferred meats and foods higher in fats and sugar. No sex differences were found for picky eating. Boys were found to have greater enjoyment for eating, with girls showing more food avoidance traits, however this may be from parental strategies reinforcing this behaviour in girls. Boys are more readily satiated and better able to regulate their food and drink intake than girls. Yet boys are also more likely to eat in the absence of hunger and have a faster eating rate and larger bite size. The paper concludes that the studies reviewed are suggestive of male–female differences in food liking and intake, appetitive traits, self-regulatory eating, and meal-related microstructure.
Abstract
The prevalence of obesity and eating disorders varies by sex, but the extent to which sex influences eating behaviors, especially in childhood, has received less attention. The purpose of this paper is to critically discuss the literature on sex differences in eating behavior in children and present new findings supporting the role of sex in child appetitive traits and neural responses to food cues. In children, the literature shows sex differences in food acceptance, food intake, appetitive traits, eating-related compensation, and eating speed. New analyses demonstrate that sex interacts with child weight status to differentially influence appetitive traits. Further, results from neuroimaging suggest that obesity in female children is positively related to neural reactivity to higher-energy-dense food cues in regions involved with contextual processing and object recognition, while the opposite was found in males. In addition to differences in how the brain processes information about food, other factors that may contribute to sex differences include parental feeding practices, societal emphasis on dieting, and peer influences. Future studies are needed to confirm these findings, as they may have implications for the development of effective intervention programs to improve dietary behaviors and prevent obesity.

Advances in medical research have resulted in prolonged survival for those with chronic diseases, making the patient’s experience vital to assessment of therapeutic effectiveness. The aim of this study was to critically appraise the measurement properties of MYMOP (Measure Yourself Medical Outcome Profile) and its adaptations. This study is a review of 16 studies evaluating four questionnaires (MYMOP and three adaptations: MYCaW, PSYCHLOPS, and MYMOP-P). Researchers reviewed the format, content and evidence of measurement properties for MYMOP, and its three adaptations. The review showed that MYMOP and its adaptations have been widely used in the evaluation of complementary therapies because of their excellent fit with individualised patient-centred approach. Therefore, individualised outcome assessment tools such as MYMOP are the way forward to personalised medicine approaches to tailor conventional therapies from patient perspective. Since validation is an ongoing process and considerable efforts have been put to develop and achieve sound psychometrics of these measures, authors conclude that researchers should further the validation of MYMOP and its adaptations before considering developing a new measure.
Abstract
BACKGROUND Health-related quality of life (HRQL) is increasingly recognized for its importance in health research. As there is increasing recognition of the inter-individual difference in response to therapeutic interventions, it may be helpful to apply individualized measures of HRQL. The MYMOP is a commonly used example of such measures, although several adaptations have been developed. OBJECTIVE This review was conducted to identify adaptations of MYMOP, and evaluate the measurement properties of MYMOP and its adaptations. METHODS Adaptations were identified using MYMOP website and personal communication, supplemented by a SCOPUS search in April 2017. Bibliographies of included studies were hand-searched. COSMIN criteria were used to evaluate the measurement properties. RESULTS Sixteen studies were included in this review. Adaptations were developed to evaluate individualized therapies in cancer, psychiatry, and acupuncture. The included measures were MYMOP, measure yourself concern and wellbeing, psychological outcome profiles (PSYCHLOPS), and MYMOP-pictorial (MYMOP-P). The quality of the measurement properties varied; none of the included measures met all currently recommended quality criteria for measurement properties. CONCLUSION Current literature provides evidence that MYMOP and its adaptations offer individualized assessment of patient-centered outcomes, and thereby provide a means to understand heterogeneity of treatment effects. However, current recommendations for psychometric testing suggest further validation of these measures would be beneficial.
