[Assessment of nutritional-dietary status, body composition, eating behavior, and perceived image in rhythmic gymnastics athletes].

Abstract
Introduction: among adolescent gymnasts some concern for body weight has been observed, with diets inadequate in energy and some nutrients in the search for maximum performance. Objective: the aim of this study was to evaluate the dietary-nutritional status, body composition, eating behavior, and perceived body image concerns of female gymnasts at sub-national competition levels. In addition, to study the relationship between the prevalence of the risk of eating disorders and variables such as substance use, irregular menstruation, fatigue, and hours of sleep. Materials and method: a total of 33 female subelite gymnasts participated (age: M = 14.52, SD = 1.85), with age ranging from 11 to 18 years. Anthropometric evaluations (restricted profile) were made, the Eating Attitudes Test (EAT-26) and Body Shape Questionnaire (BSQ) were administered, and the subjects were then interviewed for collecting sociodemographic, socio-sports, health, and food intake data. Results: the results showed that their diet was deficient, among other micro- and macro-nutrients, in water, carbohydrates, proteins, vitamins D and E, fiber, iron and calcium, whereas they ingested an excess of vitamin A and sodium. These athletes had normal weight, high muscle mass values, low fat mass levels, and intermediate somatotype components (balanced endomorph with a tendency to central). Two cases at risk for an eating disorder, and concerns related to perceived body image in a quarter of the sample were identified. Conclusion: the risk of having an eating disorder is related to the consumption of prohibited drugs or addictive substances, irregular menstruation, tiredness, and fewer hours of sleep.

Injury epidemiology and risk factors in competitive artistic gymnasts: a systematic review.

Abstract
BACKGROUND Artistic gymnastics is reported to have some of the highest injury rates in sports, which limits participation and often involves considerable medical expenses. PURPOSE To critically appraise the epidemiological literature on injury patterns and risk factors in competitive artistic gymnastics. STUDY DESIGN Systematic review. METHODS Six databases were searched for articles that investigated injuries in competitive artistic gymnasts. Injury incidence, prevalence and risk factor data were extracted, alongside information on injury location, type, severity, nature and mechanism of injury. Quality and level of evidence were assessed using a modified Downs and Black quality index checklist and the Oxford Centre for Evidence-based Medicine guidelines. RESULTS The search identified 894 articles, with 22 eligible for inclusion. Descriptive analysis showed that injury incidence and prevalence varied from 0.3 to 3.6 injuries per gymnast (female=0.3-3.6, male=0.7) and 2.0-2.3 (female=2.0-2.3, male=2.0), respectively. Male gymnasts sustained mostly upper limb injuries, while female gymnast reported lower limb injuries. Floor was associated with the greatest number of injuries for both male and female gymnasts. Higher competitive level and exposure to competition were risk factors for gymnastics injury: age, body mass, body size, training duration and life stress were significant associated factors. CONCLUSION Injury incidence and prevalence results are substantial among artistic gymnasts of all competitive levels. Gymnasts who train at highly competitive levels and are exposed to competition environments are a greater risk of injury. Future researchers should implement consistent reporting methods.

Effects of Gymnastics Activities on Bone Accrual during Growth: A Systematic Review.

Abstract
The amount of bone gained during childhood and adolescence impacts greatly on lifetime skeletal health. The purpose of this review is to summarize current evidence of the effects of gymnastics activities on bone mineral accrual during growth and to describe possible factors that influence bone mineral gains. The PubMed and SportDiscus databases were searched, and a total of 24 articles met the selection criteria and were included in this review. Artistic and rhythmic gymnasts presented higher bone mineral density and content values compared to untrained controls, despite possible negative effects associated with hormonal levels, dietary restrictions and body fat. The results suggest that gymnasts had similar bone turnover values compared to untrained controls. High-intensity mechanical loading of gymnastics activity appears to increase bone development and counterbalance negative effects, such as later pubertal development, lower body fat mass and lower hormone levels. In conclusion, gymnasts present higher bone mineral values in comparison with untrained controls. The osteogenic effect of gymnastics athletic activity has a positive influence on bone mineral accrual and overcomes the possible negative influence of high athletic activity that may cause negative energy balance and low body fat mass which are associated with lower bone accrual.

Abstract
SUMMARY Rhythmic gymnastics as high-impact bone loading sport has positive effects on bone mineralization in prepubertal years. Sclerostin and preadipocyte factor-1 (Pref-1) are hormones that inhibit bone formation. The present study demonstrates that these hormones are higher in gymnasts, and gymnasts present higher bone mineral density (BMD) as compared to controls. INTRODUCTION Rhythmic gymnasts (RG) start their heavy trainings already in prepuberty and despite of low body fat mass (FM) and hypoleptinemia, their BMD is higher than in non-trained normal girls. The specific role of sclerostin and Pref-1, which are the inhibitors of bone formation, in bone development is not well understood. The impact of sclerostin and Pref-1 levels on BMD, body composition, and adipocytokine values was studied in prepubertal RG and untrained controls (UC). METHODS Sixty-four 9-10-year-old girls were divided into RG (n = 32) and UC (n = 32) groups. Bone mineral and body composition values were measured by dual-energy X-ray absorptiometry and bone age by X-ray. Sclerostin, Pref-1, leptin, and adiponectin levels were measured from fasting blood samples. RESULTS Sclerostin (RG 19.8 ± 6.3 pmol/l; UC 15.8 ± 5.4 pmol/l) and Pref-1 (RG 1.6 ± 1.0 ng/ml; UC 1.1 ± 0.5 ng/ml) were higher (p < 0.05) in RG compared with UC. Sclerostin was related to adiponectin (r = 0.41; p < 0.05) in UC. No relationship was found between sclerostin and Pref-1 with BMD values in prepubertal RG and age-matched UC groups. CONCLUSIONS Sclerostin and Pref-1 levels are higher in RG compared to UC girls. Specific physical activity pattern seen in prepubertal RG has a beneficial effect on bone mineralization despite increased levels of hormones that inhibit bone formation.

Abstract
Short stature and later maturation of youth artistic gymnasts are often attributed to the effects of intensive training from a young age. Given limitations of available data, inadequate specification of training, failure to consider other factors affecting growth and maturation, and failure to address epidemiological criteria for causality, it has not been possible thus far to establish cause-effect relationships between training and the growth and maturation of young artistic gymnasts. In response to this ongoing debate, the Scientific Commission of the International Gymnastics Federation (FIG) convened a committee to review the current literature and address four questions: (1) Is there a negative effect of training on attained adult stature? (2) Is there a negative effect of training on growth of body segments? (3) Does training attenuate pubertal growth and maturation, specifically, the rate of growth and/or the timing and tempo of maturation? (4) Does training negatively influence the endocrine system, specifically hormones related to growth and pubertal maturation? The basic information for the review was derived from the active involvement of committee members in research on normal variation and clinical aspects of growth and maturation, and on the growth and maturation of artistic gymnasts and other youth athletes. The committee was thus thoroughly familiar with the literature on growth and maturation in general and of gymnasts and young athletes. Relevant data were more available for females than males. Youth who persisted in the sport were a highly select sample, who tended to be shorter for chronological age but who had appropriate weight-for-height. Data for secondary sex characteristics, skeletal age and age at peak height velocity indicated later maturation, but the maturity status of gymnasts overlapped the normal range of variability observed in the general population. Gymnasts as a group demonstrated a pattern of growth and maturation similar to that observed among short-, normal-, late-maturing individuals who were not athletes. Evidence for endocrine changes in gymnasts was inadequate for inferences relative to potential training effects. Allowing for noted limitations, the following conclusions were deemed acceptable: (1) Adult height or near adult height of female and male artistic gymnasts is not compromised by intensive gymnastics training. (2) Gymnastics training does not appear to attenuate growth of upper (sitting height) or lower (legs) body segment lengths. (3) Gymnastics training does not appear to attenuate pubertal growth and maturation, neither rate of growth nor the timing and tempo of the growth spurt. (4) Available data are inadequate to address the issue of intensive gymnastics training and alterations within the endocrine system.

Efficacy of five-element gymnastics in glucose and lipid control in Taiwanese patients with type 2 diabetes.

Abstract
The purpose of this quasi-experimental study was to determine the efficacy of Five-Element Gymnastics (FEG) in controlling glycosylated hemoglobin (HbA1C), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), and triglycerides (TG) at the 8th and the 16th weeks of intervention for patients with type 2 diabetes in Taiwan. FEG consolidates several traditional Chinese exercises including Qigong, Xiang Gong, and martial arts with gymnastics. The experimental group (n = 31) practiced FEG at home for 16 weeks. The control group (n = 35) maintained usual activities. FEG was associated with decrease of HbA1C, TG, and LDL-C levels at the 8th week and continuous decrease of HbA1C through the 16th week. FEG could be an exercise choice for patients with type 2 diabetes.

Abstract
Young aesthetic athletes require special understanding of the athletic biomechanical demands peculiar to each sport. The performance of these activities may impart specific biomechanical stresses and subsequent injury patterns. The clinician must understand these aspects as well as the spinal changes that occur with growth when many of these injuries often occur. Further, athletes, parents, coaches, and healthcare providers must be sensitive to the overall aspects of the athlete, including nutrition, overtraining, adequate recovery, proper technique, and limiting repetition of difficult maneuvers to minimize injuries.

Abstract
BACKGROUND Exercise training is included in cardiac rehabilitation programs to enhance physical capacity and cardiovascular function. Among the existing rehabilitation programs, exercises in water are increasingly prescribed. However, it has been questioned whether exercises in water are safe and relevant in patients with stable chronic heart failure (CHF), coronary artery disease (CAD) with normal systolic left ventricular function. The goal was to assess whether a rehabilitation program, including water-based gymnastic exercises, is safe and induces at least similar benefits as a traditional land-based training. METHODS AND RESULTS Twenty-four male CAD patients and 24 male CHF patients with stable clinical status participated in a 3-week rehabilitation. They were randomized to either a group performing the training program totally on land (CADl, CHFl; endurance + callisthenic exercises) or partly in water (CADw, CHFw; land endurance + water callisthenic exercises). Before and after rehabilitation, left ventricular systolic and cardiorespiratory functions, hemodynamic variables and autonomic nervous activities were measured. No particular complications were associated with both of our programs. At rest, significant improvements were seen in CHF patients after both types of rehabilitation (increases in stroke volume and left ventricular ejection fraction [LVEF]) as well as a decrease in heart rate (HR) and in diastolic arterial pressure. Significant increases in peaks VO(2), HR, and power output were observed in all patients after rehabilitation in exercise test. The increase in LVEF at rest, in HR and power output at the exercise peak were slightly higher in CHFw than in CHFl. CONCLUSIONS Altogether, both land and water-based programs were well tolerated and triggered improvements in cardiorespiratory function.

Calcium supplementation and weight bearing physical activity--do they have a combined effect on the bone density of pre-pubertal children?

Abstract
The adaptation of bone to exercise has been shown to be modified by dietary calcium intake. The aim of this randomised controlled trial was to investigate whether there was a differential response to calcium supplementation in elite gymnasts and school children controls. The primary hypothesis was that gymnasts who took calcium supplements would have greater increases in cortical and trabecular volumetric bone mineral density (vBMD) at the radius and tibia. Secondary outcomes studied were changes in bone geometry at the radius and tibia and lumbar spine and whole body measurements. Children were randomised to 12 months daily supplementation of 500 mg elemental calcium (1250 mg (in the form of calcium carbonate salt)) or placebo. Outcome measures were assessed using peripheral quantitative computed tomography (pQCT) (distal and diaphyseal radius and tibia) and dual energy X-ray absorptiometry (DXA) (lumbar spine and whole body). Eighty-six subjects participated in the trial (44 gymnasts, 42 controls) and 75 subjects completed the trial (39 gymnasts, 36 controls). Data were analysed by analysis of covariance adjusting for baseline value of bone parameters, age, height, gender and puberty, and delay between baseline measurement and start of intervention. The primary analysis was for a calcium-exercise interaction; a pooled calcium effect with no interaction was also tested. Results are presented as ratios (95% confidence intervals). At the distal tibia, trabecular vBMD showed a significant interaction (p=0.04), with controls (1.00: 0.99, 1.09) responding more than gymnasts (0.98: 0.94, 1.02) to supplementation. At the distal radius, change in trabecular vBMD was not significant (p=0.05). There were no differences in change in cortical vBMD at either site between the gymnasts and controls (tibia: p=0.82, radius: p=0.88). For all other secondary outcomes at radius, tibia, spine and whole body no significant interactions were found. In conclusion, there was no beneficial effect of additional calcium in gymnasts who already consume their recommended nutrient intake (888 mg/day; United Kingdom reference nutrient intake for 8- to 11-year-olds is 555-800 mg/day) for calcium. We speculate that gymnasts have already adapted their bones (geometry and vBMD) to the demands imposed upon them by the loading they are subjected to during gymnastics and do not benefit from additional calcium supplementation.

Training does not affect protein turnover in pre- and early pubertal female gymnasts.

Abstract
This study compared protein turnover in ten young female gymnasts [10.3 (0.5) years] engaged in regular intense physical training with ten age-matched controls [9.4 (0.6) years)]. Nitrogen flux ( Q), protein synthesis (PS), protein degradation (PD) and net protein turnover (NPB = PS-PD) were measured following a single oral dose of [(15)N]-glycine. The habitual dietary intake of each subject was assessed using a 7-day food record, with food portions being weighed before ingestion. The gymnasts had a low total energy intake which was unbalanced in the proportions of lipid, carbohydrate and protein. Protein flux was 7.19 (0.35) g.kg(-1).day(-1) in the gymnasts and 7.53 (0.81) g.kg(-1).day(-1) in the controls; protein synthesis was 6.06 (0.27) g.kg(-1).day(-1 )in the gymnasts and 6.53 (0.74) g.kg(-1).day(-1) in the controls; protein degradation was 5.45 (0.38) g.kg(-1).day(-1) in the gymnasts and 5.27 (0.74) g.kg(-1).day(-1) in the controls. All data are presented as means and standard errors of the mean (SEM). There were no statistical differences for protein flux, protein synthesis or protein degradation between the two groups. However, NPB was lower (-14%) in the trained gymnasts than in the control group ( P <0.05), which might be explained by a greater protein ingestion in the control group on the day of the protocol ( P <0.05). These results show that in pre- and early pubertal female gymnasts intense training does not exert a demonstrable effect on protein turnover.
