Serum Chromium Levels Are Higher in Peritoneal Dialysis than in Hemodialysis Patients.

Abstract
Background:An elevation in serum chromium levels in individuals treated with renal replacement therapy has been previously described, but chromium levels have not been systematically studied in patients treated with different dialysis modalities. The aim of this study was to compare serum chromium levels in patients treated with chronic peritoneal dialysis (PD) and hemodialysis (HD).Methods:We studied 169 chronic dialysis patients in a single medical center, of which 148 were treated with HD and 21 with PD. Serum chromium levels were measured by atomic absorption spectrometry.Residual renal function was accessed using a timed urine collection for the measurement of urine output and calculation of glomerular filtration rate (GFR).Results:The median (interquartile range) serum chromium level was significantly higher in patients treated with PD than in patients treated with HD: 5.00 (3.24 - 6.15) vs 1.83 (1.29 - 2.45) mcg/L, p < 0.001. In a univariate analysis, serum chromium level was associated with PD modality: Exp (B) 7.46 (95% confidence interval [CI] 2.1 - 26.4), p = 0.002. The association of PD modality with serum chromium level was even more significant using a multivariate logistic regression model: odds ratio (OR) 11.87 (95% CI 2.85 - 49.52), p = 0.001 after adjustment for age, gender, diabetes, smoking, dialysis vintage, use of diuretics, and residual renal function.Conclusions:In patients treated with chronic dialysis, serum chromium levels are higher in patients treated with PD than in those treated with HD.

Comparison of Whole-Blood Metal Ion Levels Among Four Types of Large-Head, Metal-on-Metal Total Hip Arthroplasty Implants: A Concise Follow-up, at Five Years, of a Previous Report.

Abstract
Few studies of total hip arthroplasty (THA) implants with a large-diameter femoral head and metal-on-metal design have directly compared the progression of metal ion levels over time and the relationship to complications. As we previously reported, 144 patients received one of four types of large-diameter-head, metal-on-metal THA designs (Durom, Birmingham, ASR XL, or Magnum implants). Cobalt, chromium, and titanium ion levels were measured over five years. We compared ion levels and clinical results over time. The Durom group showed the highest levels of cobalt (p ≤ 0.002) and titanium ions (p ≤ 0.03). Both the Durom and Birmingham groups demonstrated significant ongoing cobalt increases up to five years. Eight patients (seven with a Durom implant and one with a Birmingham implant) developed adverse local tissue reaction. Six Durom implants and one Birmingham implant required revision, with one pseudotumor under surveillance at the time of the most recent follow-up. We found that ion generation and related complications varied among designs. More concerning was that, for some designs, ion levels continued to increase. Coupling a cobalt-chromium adapter sleeve to an unmodified titanium femoral trunnion along with a large metal-on-metal bearing may explain the poor performances of two of the designs in the current study.

Bonding Strength of Ceromer with Direct Laser Sintered, Ni-Cr-Based, and ZrO2 Metal Infrastructures After Er:YAG, Nd:YAG, and Ho:YAG Laser Surface Treatments-A Comparative In Vitro Study.

Abstract
OBJECTIVE Laser modalities instead of conventional surface treatment techniques have been suggested to obtain an adequate micromechanical bonding between dental super- and infrastructures. The present study was undertaken to assess the effect of surface treatment with Ho:YAG, Er:YAG, and Nd:YAG laser modalities on the shear bond strength (SBS) of ceromer to different types of metal infrastructures in in vitro settings. METHODS The study specimens consisted of 40 direct laser sintered (DLS), 40 Ni-Cr-based, and 40 zirconium oxide (ZrO2) infrastructures. In each infrastructure group, the specimens were divided randomly into five treatment modalities (n = 8): no treatment (controls), sandblasting, Er:YAG, Nd:YAG, and Ho:YAG lasers. The DLS, Ni-Cr-based, and ZrO2 infrastructures were prepared in the final dimensions of 7 mm in diameter and 3 mm in thickness in line with the ISO 11405 standard. Ceromer as superstructure was applied to all the infrastructures after their surface treatments according to the selected treatment modality. SBS test was performed to test the effectiveness of surface treatments. A stereomicroscope was used to determine the changes in the surface morphology of specimens. RESULTS Among the laser modalities and sandblasting, Ho:YAG laser caused the most important increase in the DLS and Ni-Cr-based infrastructures but sandblasting caused the most important increase in the ZrO2 infrastructure. In all the infrastructures, Nd:YAG laser has the least effectiveness, and Er:YAG laser makes an intermediate success. The stereomicroscopy images presented that the applications of laser surface treatments altered the surface in all the infrastructures. CONCLUSIONS Overall, in current experimental settings, Ho:YAG, Nd:YAG, and Er:YAG lasers, in order of strength, are effective in improving the bonding of ceromer to all the infrastructures. Ho:YAG laser is more effective in the DLS and Ni-Cr-based infrastructures, but sandblasting is more effective in the ZrO2 infrastructure. The studied laser modalities need to be subjected for further studies to determine their appropriate settings and the specifications to be used in prosthodontics.

[Clinical assessment of dietary correction of metabolic syndrome by using specialized food product enriched with chrome].

Abstract
Effectiveness of Laminaria jam enriched with chromium has been investigated in ambulatory patients with metabolic syndrome. 50 patients ranging in age from 30 to 65 years (mean age 53.4 +/- 7.5) were under observation during 6 months. 30 patients from comparison group were given recommendations on modeling of lifestyle (diet, reduced and controlled dosed physical loading) and they received medical treatment. Main group of patients (20 people) in addition received dietary product--Laminaria jam, enriched with chromium, which daily dose (20 g) contain 280 microg of iodine and 66 microg of chromium. In both groups declines of overweight, improving clinical and laboratory parameters, adaptive organism reserves and quality of life has been registered. Patients from the main group referred good organoleptic properties of dietary product, it was well tolerated by patients without causing unwanted side effects. After 6 months of a dietary correction mean relative body mass reduction in the main group amounted to 5.0% while in the comparison group--3.0%. Positive dynamics of the circumference of the waist in patients accounted for 5.9% in main group and 3.1% in comparison group. Systolic and diastolic blood pressure decreased in the main group, an average of 20.5 and 24.2%; in the comparison group--10.5 and 13.3% respectively. The average reduction in LDL-C reached 28.3% in the main group and 18.3% in the comparison group. Triglyceride levels decrease was an average of 18.9% in the studied group, that was significantly higher than in the control group (3.7%). The dynamics of OGTT in main group amounted to 13.1% while in the comparison group--8.6%. Differences between group indices were statistically significant for most of the variables. The inclusion of the dietary product in a range of therapeutic and preventive activities under metabolic syndrome has been shown to improve clinical and metabolic indicators (body mass, arterial pressure, lipid and carbohydrate metabolism parameters) compared with standard therapy.

Cushion bearings versus large diameter head metal-on-metal bearings in total hip arthroplasty: a short-term metal ion study.

Abstract
INTRODUCTION Metal-on-metal total hip arthroplasty (MOM THA) has the advantage of replicating the femoral head size, but the postoperative elevation of serum metal ion levels is a cause for concern. Metal-on-polycarbonate-urethane is a new cushion bearing featuring a large diameter metal head coupled with a polycarbonate-urethane liner. AIM: The aim of this study was to assess and compare serum cobalt (Co) and chromium (Cr) levels in a group of 15 patients treated with a cushion bearing THA system (Group A) and a group of 15 patients treated with a MOM THA system (Group B) at short-term. At a mean follow-up of 27.3 months (18-35 months), in Group A the median Cr and Co serum levels were significantly lower than in Group B, measuring 0.24 μg/L (0.1-2.1 μg/L) and 0.6 μg/L (0.29-2.3 μg/L) compared to 1.3 μg/L (0.1-9 μg/L, p < 0.001) and 2.9 μg/L (0.85-13.8 μg/L, p < 0.001) respectively. RESULTS All patients demonstrated an excellent clinical result, as shown by the Harris and Oxford hip scores. The cushion bearing THA studied in this paper showed clinical outcomes similar to the MOM THA bearing, with the advantage of no significant metal ion elevation in the serum. CONCLUSION These findings warrant the continued clinical study of compliant bearing options.

Abstract
In a double-blinded randomised controlled trial, 83 patients with primary osteoarthritis of the hip received either a ceramic-on-metal (CoM) or metal-on-metal (MoM) total hip replacement (THR). The implants differed only in the bearing surfaces used. The serum levels of cobalt and chromium and functional outcome scores were compared pre-operatively and at six and 12 months post-operatively. Data were available for 41 CoM and 36 MoM THRs (four patients were lost to follow-up, two received incorrect implants). The baseline characteristics of both cohorts were similar. Femoral head size measured 36 mm in all but two patients who had 28 mm heads. The mean serum cobalt and chromium levels increased in both groups, with no difference noted between groups at six months (cobalt p = 0.67, chromium p = 0.87) and 12 months (cobalt p = 0.76, chromium p = 0.76) post-operatively. Similarly, the mean Oxford hip scores, Western Ontario and McMaster Universities Osteoarthritis index and University of California, Los Angeles activity scores showed comparable improvement at 12 months. Our findings indicate that CoM and MoM couplings are associated with an equivalent increase in serum cobalt and chromium levels, and comparable functional outcome scores at six and 12-months follow-up.

Chromium, zinc and magnesium concentrations in the pubic hair of obese and overweight women.

Abstract
The study addressed chromium, zinc and magnesium concentrations in the pubic hair of obese and overweight women. It was carried out on hair collected from 85 women at the age of 16-80 living in the Podkarpackie Voivodeship (southern Poland). The experimental and control groups consisted of 39 and 46 females, respectively. The pubic hair was prepared under a procedure established by the International Atomic Energy Agency, followed by wet digestion in a microwave oven. The concentration of the metals in the pubic hair and reference material was assayed with the flame (Mg, Zn) and flameless (Cr) atomic absorption spectrometry. The pubic hair of overweight and obese women from the experimental group revealed significantly higher chromium and magnesium concentrations and significantly lower concentrations of zinc than in the control group. An increase in BMI brought about an increase in chromium and magnesium concentrations while zinc concentration decreased with increasing BMI. The disturbances in the mineral balance of overweight and obese women were also demonstrated by significantly different ratios of the elements compared with the control group.

Abstract
We have updated our previous randomised controlled trial comparing release of chromium (Cr) and cobalt (Co) ions and included levels of titanium (Ti) ions. We have compared the findings from 28 mm metal-on-metal total hip replacement, performed using titanium CLS/Spotorno femoral components and titanium AlloFit acetabular components with Metasul bearings, with Durom hip resurfacing using a Metasul articulation or bearing and a titanium plasma-sprayed coating for fixation of the acetabular component. Although significantly higher blood ion levels of Cr and Co were observed at three months in the resurfaced group than in total hip replacement, no significant difference was found at two years post-operatively for Cr, 1.58 microg/L and 1.62 microg/L respectively (p = 0.819) and for Co, 0.67 microg/L and 0.94 microg/L respectively (p = 0.207). A steady state was reached at one year in the resurfaced group and after three months in the total hip replacement group. Interestingly, Ti, which is not part of the bearing surfaces with its release resulting from metal corrosion, had significantly elevated ion levels after implantation in both groups. The hip resurfacing group had significantly higher Ti levels than the total hip replacement group for all periods of follow-up. At two years the mean blood levels of Ti ions were 1.87 microg/L in hip resurfacing and and 1.30 microg/L in total hip replacement (p = 0.001). The study confirms even with different bearing diameters and clearances, hip replacement and 28 mm metal-on-metal total hip replacement produced similar Cr and Co metal ion levels in this randomised controlled trial study design, but apart from wear on bearing surfaces, passive corrosion of exposed metallic surfaces is a factor which influences ion concentrations. Ti plasma spray coating the acetabular components for hip resurfacing produces significantly higher release of Ti than Ti grit-blasted surfaces in total hip replacement.

Abstract
The structure of the 24 kDa cysteine protease saru-actinidin from the fruit of Actinidia arguta Planch. (sarunashi) was determined by the cadmium/sulfur-SAD method with X-ray diffraction data collected using in-house Cu Kα and Cr Kα radiation. The anomalous scatterers included nine sulfurs and several cadmium ions from the crystallization solution. The high quality of the diffraction data, the use of chromium-anode X-ray radiation and the substantial anomalous signal allowed structure determination and automated model building despite both a low solvent content (<40%) and low data multiplicity. The amino-acid sequence of saru-actinidin was deduced from the cDNA and was modified based on experimental electron-density maps at 1.5 Å resolution. The active site of saru-actinidin is occupied by a cadmium ion and the active-site cysteine is found to be in an unmodified, cysteine sulfenic acid or cysteine sulfinic acid form. The cadmium sites, coordination geometries and polygonal water structures on the protein surface have also been extensively analyzed. An analysis and comparison of the sulfur/cadmium anomalous signals at the Cu Kα and Cr Kα wavelengths was carried out. It is proposed that the inclusion of cadmium salts in crystallization solutions coupled with chromium-anode radiation can provide a convenient route for structure determination.

Modelling Cr(VI) removal by a combined carbon-activated sludge system.

Abstract
The combined carbon-activated sludge process has been proposed as an alternative to protect the biomass against toxic substances in wastewaters; however, the information about the effect of powdered-activated carbon (PAC) addition in activated sludge reactors for the treatment of wastewaters containing Cr(VI) is limited. The objectives of the present study were: (a) to evaluate the removal of hexavalent chromium by (i) activated sludge microorganisms in aerobic batch reactors, (ii) powdered-activated carbon, and (iii) the combined action of powdered-activated carbon and biomass; (b) to propose mathematical models that interpret the experimental results. Different Cr(VI) removal systems were tested: (S1) biomass (activated sludge), (S2) PAC, and (S3) the combined activated carbon-biomass system. A Monod-based mathematical model was used to describe the kinetics of Cr(VI) removal in the system S1. A first-order kinetics with respect to Cr(VI) and PAC respectively, was proposed to model the removal of Cr(VI) in the system S2. Cr(VI) removal in the combined carbon-biomass system (S3) was faster than both Cr(VI) removal using PAC or activated sludge individually. Results showed that the removal of Cr(VI) using the activated carbon-biomass system (S3) was adequately described by combining the kinetic equations proposed for the systems S1 and S2.
