Effect of oral isotretinoin on muscle strength in patients with acne vulgaris: a prospective controlled study.

Abstract
BACKGROUND Musculoskeletal side effects related to isotretinoin are frequently reported. This study aimed to investigate the effect of oral isotretinoin treatment on muscle strength. Our second aim was to evaluate whether there was a correlation between the serum creatine phosphokinase (CPK) level, a specific marker of muscle breakdown, and muscle strength. METHODS This study included 30 patients who presented to our hospital and were started on oral isotretinoin treatment for acne vulgaris and 30 patients in the control group who were given local treatment. Age, sex, height and weight of the patients were recorded, and the body mass index (BMI) was calculated. The hamstring and quadriceps muscle strengths of the non-dominant side were evaluated in all patients using an isokinetic dynamometer, and the peak torque (PT) values were recorded. In the isotretinoin group, isokinetic measurements were performed again in those that completed six-month drug treatment and compared with the initial PT values. RESULTS The two groups were similar in terms of age, sex, and BMI (p > 0.05). There was no significant difference between the isotretinoin and control groups in terms of muscle strength at the beginning of the treatment (p > 0.05). No significant change was observed in hamstring and quadriceps PT values in the isotretinoin group after 6 months of treatment compared to baseline (p > 0.05). No statistically significant correlation was found between the serum CPK level and hamstring and quadriceps muscle strength (p > 0.05). CONCLUSION Oral isotretinoin doesn't alter muscle strength. There is no relationship between the serum CPK levels and muscle strength.

The acute effects of decaffeinated versus caffeinated coffee on reaction time, mood and skeletal muscle strength.

Abstract
Background Caffeinated coffee, a psycho-stimulant, is widely consumed throughout the globe. However, its chronic consumption has deleterious effects on human health. Meanwhile, decaffeinated coffee has low content of caffeine and thus can be an alternative to caffeinated coffee. Therefore, the study was undertaken to explore and compare the acute effects of decaffeinated and caffeinated coffee on reaction time, mood and skeletal muscle strength in healthy volunteers. Methods This was a prospective, interventional, comparative type of study. The study included 70 healthy adults divided into two groups (Caffeinated coffee group and Decaffeinated coffee group). The following parameters were assessed: reaction time was assessed by digital display multiple-choice apparatus, mood by Visual Analogue Scale (VAS) and Profile of Mood States revised version (POMS) and skeletal muscle strength by hand dynamometer. All parameters in both groups were assessed pre-intervention (baseline) and 30 min post-intervention. Results In both groups (decaffeinated and caffeinated coffee) post-intervention, there was a statistically significant (p < 0.05) improvement in the reaction time (VRT) and mood (VAS, POMS) from the baseline. However, both groups did not show any significant effects on the skeletal muscle strength. Upon comparing the two groups, we found that caffeinated coffee showed higher and significant improvement of mood than decaffeinated coffee. Conclusions Decaffeinated coffee exerts an acute significant stimulatory effect on the reaction time and mood. However, these effects in comparison to caffeinated coffee are low. Further randomized control clinical trials are thus needed to validate these interesting findings.

Eleven Weeks of Iron Supplementation Does Not Maintain Iron Status for an Entire Competitive Season in Elite Female Volleyball Players: A Follow-Up Study.

Abstract
BACKGROUND Even though iron supplementation can be effective, it is necessary to be cautious of toxicity and aim to do no harm, therefore, it is important to examine the length of time the benefits of iron supplementation can be maintained following its cessation. The main purpose of this study was to analyze if iron stores and strength performance were maintained in elite female volleyball players for the final 18 weeks of a competitive season following the cessation of 11 weeks of iron supplementation. METHODS Twenty-two volleyballers (age: 27.0 ± 5.6 years.) were assigned to two groups (iron treatment group-ITG, n = 11 or control gropu-CG, n = 11) at the beginning of a previous trial (T0) and ITG consumed 325mg/d of ferrous sulphate for 11 weeks (T11). Then, in the present study iron status and strength were measured again 10 (T21) and 18 weeks later (T29) after the cessation of supplementation. RESULTS At the end of the previous trial (T11), ITG maintained iron status as measured by hematological parameters (serum iron-sFE, serum ferritin-FER, transferrin saturation index-TSI, and hemogloblin-Hb), however, CG showed a decrease in these markers at T11. Further, from T0 to T11 ITG experienced greater (p < 0.05) changes in clean and jerk, power clean, and total mean strength (TMS-sum of all strength tests) than CG. In the present, follow-up investigation, there was a group-by-time interaction in favor of CG vs. ITG from T11 to T21 for FER (p = 0.028) and Hb (p = 0.042). Further, there was an increase for CG (p < 0.001) in power clean for CG from T11 (38.4 ± 1.7 kg) to T21 (41.3 ± 1.9 kg) and T29 (41.8 ± 1.7 kg), but no change for power clean in ITG (p > 0.05). A group-by-time interaction from T11 to T29 occurred in favor of CG for half-squat (p = 0.049) and TMS (p = 0.049). CONCLUSION Our findings suggest that the benefits of iron supplementation are not sustained in elite female volleyballers if supplementation is ceased for 18 weeks.

Higher DHEAS Levels Associated with Long-Term Practicing of Tai Chi.

Abstract
Tai Chi has many benefits for middle-aged/older individuals including improvements to muscle strength and various body lipid components. DHEAS and testosterone have anti-obesity/anti-aging characteristics and also improve libido, vitality and immunity levels. Thus, the aim of the present study was to investigate the differences between middle-aged Tai Chi practitioners (n = 17) and sedentary individuals (n = 17) in terms of leg strength, blood levels of cholesterol, triglyceride, HDL, as well as DHEAS, testosterone and cortisol. Unpaired t-tests were used to identify significant differences between the two groups. There were no significant differences in body composition, leg strength, blood lipid components and testosterone. However, the Tai Chi practitioners had higher levels of DHEAS (P < 0.01) and lower levels of cortisol (P < 0.05). Thus, Tai Chi practitioners have a higher ratio of DHEAS to cortisol, which might have potential benefits in terms of improving an individual’s health-related quality of life during the aging.

Brain-derived neurotrophic factor correlated with muscle strength in subjects undergoing stationary bicycle exercise training.

Abstract
AIMS: Several central nervous disorders are associated with metabolic syndrome (MetS) and type 2 diabetes. Reduction in brain-derived neurotrophic factor (BDNF) is involved in the mechanism of central nervous dysfunction. BDNF is up-regulated after exercise, but it is not known whether increased BDNF is related to increases in muscle strength. METHODS In the present study, subjects with MetS or type 2 diabetes were enrolled in an exercise program. All participants underwent an indoor bicycle exercise program for twelve weeks. Serum BDNF was determined after overnight fasting. Muscle strength was assessed by extension of the dominant lower extremity. RESULTS A total of 33 subjects were enrolled in this study. The body mass index did not change significantly (from 30.4±6.0 to 30.2±5.8kg/m(2), P=0.436), but serum BDNF increased significantly (from 17.1±9.1 to 24.2±10.7ng/mL, P<0.001) after the study. The exercise-associated BDNF was significantly correlated with the increased strength in lower-extremity extension test (r=0.54, P=0.001). Using multivariate regression analysis, muscle-strength increment, but not body-weight change, was an independent factor for serum BDNF (95% CI=0.009-0.044, P=0.005). CONCLUSIONS After a twelve-week program of stationary bicycle exercise, serum BDNF concentration increased, and this change was positively correlated with muscle strength of lower-extremity extension, but not body weight. A 3-year school-based exercise intervention improves muscle strength - a prospective controlled population-based study in 223 children.

Abstract
BACKGROUND Intense physical activity (PA) improves muscle strength in children, but it remains uncertain whether moderately intense PA in a population-based cohort of children confers these benefits. METHODS We included children aged 6-9 years in four schools where the intervention school increased the school curriculum of PA from 60 minutes/week to 40 minutes/school day while the control schools continued with 60 minutes/week for three years. We measured muscle strength, as isokinetic Peak Torque (PT) (Nm) of the knee flexors in the right leg at speeds of 60°/second and 180°/second, at baseline and at follow-up, in 47 girls and 76 boys in the intervention group and 46 girls and 54 boys in the control group and then calculated annual changes in muscle strength. Data are provided as means with 95% confidence intervals. RESULTS Girls in the intervention group had 1.0 Nm (0.13, 1.9) and boys 1.9 Nm (0.9, 2.9) greater annual gain in knee flexor PT at 60°/second, than girls and boys in the control group. Boys in the intervention group also had 1.5 Nm (0.5, 2.5) greater annual gain in knee flexors PT at 180°/second than boys in the control group. CONCLUSION A 3-year moderately intense PA intervention program within the school curriculum enhances muscle strength in both girls and boys.

Muscle strength, motor performance, cardiac and muscle biomarkers in detection of muscle side effects during and after acute lymphoblastic leukemia treatment in children.

Abstract
Muscle side effects have not been frequently assessed in childhood acute lymphoblastic leukemia (ALL) patients. Our objective was to determine the early and late muscle side effects during childhood ALL treatment. To this end, we examined the early muscle side effects in 15 newly diagnosed "therapy patients" (group I), and the late side effects in 18 ALL survivors "off therapy patients" (group II). Muscle side effects were assessed by measuring hand grip strength (HGS), the "timed up and go" (TUG) test, creatine phosphokinase, myoglobin, plasma electrolytes, cardiac troponin I before and after induction chemotherapy in group I. The same parameters and cranial radiotherapy dose were examined in group II at a single timepoint. Cumulative doses of chemotherapy were calculated, and echocardiograms were obtained for each group. As a result, we found that the HGS and TUG measures of group I at the end of induction were poorer than measures of the first admission, control subjects, and group II. Low HGS measures had been normalized over time, but the TUG measures remained poor. Overt rhabdomyolysis and pyomyositis were not detected in any of the patients. These results suggested that muscle side effect monitoring might be helpful in the follow-up of children with ALL.

The influence of winter vitamin D supplementation on muscle function and injury occurrence in elite ballet dancers: a controlled study.

Abstract
OBJECTIVES Athletes who train indoors during the winter months exhibit low serum 25-hydroxyvitamin D [25(OH)D] concentrations due to a lack of sunlight exposure. This has been linked to impaired exercise performance. The purpose of this study was to assess the effects of oral vitamin D₃ supplementation on selected physical fitness and injury parameters in elite ballet dancers. DESIGN Controlled prospective study. METHODS 24 elite classical ballet dancers (intervention n=17; control n=7) participated in a controlled 4-month oral supplementation of vitamin D₃ (2000 IU per day). Isometric muscular strength and vertical jump height were measured pre and post intervention. Injury occurrence during the intervention period was also recorded by the in-house medical team. Repeated measures ANOVA and Mann-Whitney-U statistical tests were used and significance was set at p ≤ 0.05. RESULTS Significant increases were noted for the intervention group for isometric strength (18.7%, p<0.01) and vertical jump (7.1%, p<0.01). The intervention group also sustained significantly less injuries than the controls during the study period (p<0.01). CONCLUSIONS Oral supplementation of vitamin D₃ during the winter months has beneficial effects on muscular performance and injury occurrence in elite ballet dancers.

Aerobic exercise and respiratory muscle strength in patients with cystic fibrosis.

Abstract
BACKGROUND The beneficial role of exercise in maintaining health in patients with cystic fibrosis (CF) is well described. Few data exist on the effect of exercise on respiratory muscle function in patients with CF. Our objective was to compare respiratory muscle function indices in CF patients that regularly exercise with those CF patients that do not. METHODS This cross-sectional study assessed nutrition, pulmonary function and respiratory muscle function in 37 CF patients that undertook regular aerobic exercise and in a control group matched for age and gender which consisted of 44 CF patients that did not undertake regular exercise. Respiratory muscle function in CF was assessed by maximal inspiratory pressure (Pimax), maximal expiratory pressure (Pemax) and pressure-time index of the respiratory muscles (PTImus). RESULTS Median Pimax and Pemax were significantly higher in the exercise group compared to the control group (92 vs. 63 cm H2O and 94 vs. 64 cm H2O respectively). PTImus was significantly lower in the exercise group compared to the control group (0.089 vs. 0.121). Upper arm muscle area (UAMA) and mid-arm muscle circumference were significantly increased in the exercise group compared to the control group (2608 vs. 2178 mm2 and 23 vs. 21 cm respectively). UAMA was significantly related to Pimax in the exercising group. CONCLUSIONS These results suggest that CF patients that undertake regular aerobic exercise maintain higher indices of respiratory muscle strength and lower PTImus values, while increased UAMA values in exercising patients highlight the importance of muscular competence in respiratory muscle function in this population.

Does extensive on-water rowing increase muscular strength and endurance?

Abstract
The purpose of this study was to compare changes in aerobic condition, strength, and muscular endurance following 8 weeks of endurance rowing alone or in combination with weight-training. Twenty-two elite rowers were assigned to (1) rowing (n = 10, 250-270 km · week⁻¹) or (2) rowing (n = 12, 190-210 km · week⁻¹) plus four weight-training sessions each week. Pre and post mean and standardized effect-size (ES) differences in aerobic condition (watts at 4 mmol · L⁻¹) and strength (isometric pull, N), prone bench-pull (6-repetition maximum, 6-RM), 5- and 30-repetition leg-press and 60-repetition seated-arm-pull (J, performed on a dynamometer) normalized by body mass and log-transformed were analysed, after adjusting for gender. The standardized differences between groups were trivial for aerobic condition (ES [±90% CI] = 0.15; ±0.28, P = 0.37) and prone bench-pull (ES = 0.27; ±0.33, P = 0.18), although a moderate positive benefit in favour of rowing only was observed for the seated-arm-pull (ES = 0.42; ±0.4, P = 0.08). Only the weight-training group improved isometric pull (12.4 ± 8.9%, P < 0.01), 5-repetition (4.0 ± 5.7%, P < 0.01) and 30-repetition (2.4 ± 5.4%, P < 0.01) leg-press. In conclusion, while gains in aerobic condition and upper-body strength were comparable to extensive endurance rowing, weight-training led to moderately greater lower-body muscular-endurance and strength gains.
